Consent to the processing of personal data

In accordance with the Federal Law No. 152-FZ dated July 27, 2006, "On Personal Data", I hereby confirm that by using the website https://unisky.aero/ (hereinafter referred to as the "Website") or by providing my personal data on the Website or by phone, I am a personal data subject and also a lawful representative of other personal data subjects from whom I have obtained consent to process their personal data and to transfer such data to third parties.  
   
I, on my own behalf, of my own free will and in my own interests (including on behalf, will, and in the interests of other personal data subjects whom I lawfully represent), hereby give consent to LLC "UNI SKY", OGRN 1227700867679, INN 9731106136, legal address: 121354, Moscow, Dorogobuzhskaya St., 14, Bldg. 4 (hereinafter referred to as the **OPERATOR), to process the following personal data of mine: surname, name, patronymic, gender, date of birth, place of residence (city, region), residential address; phone numbers, email addresses, personal data subjects’ addresses, number of the primary identity document and identity documents of other personal data subjects, information about the issue date of such document and the issuing authority, passport data for international travel, point of departure and destination, travel date, dietary preferences, link to social media profile on VK.com; Telegram username; IP address, cookies, browser information, access time, address of the visited page, previous page address; statistics related to my IP address, activity on the OPERATOR’s Website (date, time, and duration of activity, activity history including transitions between pages and hyperlinks on the Website, page view rate).

By providing this information, I guarantee that the personal data is accurate, that I lawfully possess it, and I agree to provide supporting documents upon the OPERATOR’s request.

I hereby give my consent to the performance of any action or set of actions with the aforementioned personal data as prescribed by Federal Law No. 152-FZ dated July 27, 2006, "On Personal Data", with or without the use of automation tools, including the collection, recording, systematization, accumulation, storage, clarification (updating, modification), extraction, use, transfer (distribution, provision, access), including blocking, deletion, and destruction of personal data. The purpose of such processing is:

• ⁠fulfillment of agreements related to access to the Website, its content and/or services, and functionality;
• ⁠identification during registration on the Website, completion of the “Feedback” form, and/or while using the service;
• booking and provision of services, processing of inquiries and requests, verification of request status;
• establishing feedback, including sending notifications and requests, concluding a service contract for flight arrangements;
• confirmation of the completeness of the provided personal data;
• settlement of payments;
• OPERATOR's collection of statistical data;
• ⁠improving the quality and usability of the Website and/or its services, and the development of new services.

I hereby acknowledge and confirm that the OPERATOR is entitled to entrust the processing of my personal data and that of the subjects I represent to third parties under a concluded agreement, when necessary for providing flight arrangement services, including but not limited to airlines, postal services, Internet service providers, and email service providers.

The individuals processing personal data on behalf of the OPERATOR shall comply with the principles and rules of data processing and protection in accordance with Federal Law No. 152-FZ dated July 27, 2006, and ensure confidentiality and security of personal data. If such third parties fail to comply, they shall bear liability as per their contractual obligations with the OPERATOR and/or applicable data protection legislation.

By ticking the checkbox "I am familiar with and consent to the processing of personal data", I confirm that:

• ⁠I give the OPERATOR my specific, informed, deliberate, and unequivocal consent for the purposes described above;
• The personal data provided by me to the OPERATOR and/or its authorized representatives is accurate, complete, and up-to-date at the time of provision;
• The consent (or a copy thereof) and/or circumstances of its provision may be disclosed by the OPERATOR to third parties as proof of receiving my consent.

This consent is granted by me personally and remains valid until revoked by submitting a signed written statement to the OPERATOR via email at info@unisky.aero.  

Upon receipt of my written revocation of this consent, the OPERATOR shall cease processing the personal data to the extent of its collection, recording, systematization, accumulation, clarification (updating, modification), extraction, use, or transfer (distribution, access).

By providing this consent, I confirm that I and the personal data subjects specified by me on the website https://unisky.aero/ and in the agreement concluded with the OPERATOR, have read and understood the OPERATOR’s Personal Data Processing and Protection Policy, this Consent to Personal Data Processing, and the Federal Law No. 152-FZ dated July 27, 2006, "On Personal Data", and accept the terms of the User Agreement.

In case of any discrepancy in the data provided in this Consent, including the absence of confirmation of lawful possession of data provided to me by other data subjects, I undertake to stop using the Website and refrain from concluding contracts on behalf of third parties.
